Part of the Recovery Roadmap Tip series, this is a 25-item quality review tool to help people in recovery/families to reflect on the extent to which their planning PROCESS (meetings/conversations, etc.) reflects certain person-centered practices and content. The tool helps people/families to reflect on whether or not they are receiving good person-centered planning. They can use this tool to identify things that are going well, and things that might need to be improved. Users are encouraged you to discuss their feedback with their teaam or individual service provider.
