Newport Mental Health Person-Centered Planning Audit and Quality Monitoring Tool

This 13-item auditing tool is designed to monitor the quality of person centered recovery plans. Developed and implemented by Newport Mental Health, a comprehensive community based mental health center and CCBHC, the items include evidence of the importance of:

  • input from the person,
  • presence and quality of life goals that “drive he plan,”
  • description of problems as barriers to attainment of life goals that satisfy medical necessity requirements, and
  • well articulated, measurable objectives – integration of strengths into the plan.
