This 13-item auditing tool is designed to monitor the quality of person centered recovery plans. Developed and implemented by Newport Mental Health, a comprehensive community based mental health center and CCBHC, the items include evidence of the importance of:
- input from the person,
- presence and quality of life goals that “drive he plan,”
- description of problems as barriers to attainment of life goals that satisfy medical necessity requirements, and
- well articulated, measurable objectives – integration of strengths into the plan.
