Positive Rituals Survey: Person Centered Tools and Practices:

Person-centered thinking tool to capture information about what rituals and routines help to create a positive experience and good day for a person. Rituals can be selected from a list or others can be added that are important to a person. Rituals can include daily activities (walking a dog), consistent activities (weekly garden club), or less frequent, but important activities such as family or holiday rituals. A rich description of valued rituals and routines can be used to help connect people to naturally occuring community relationships and activities.
