Intervention/Action Steps Tip Sheet

Review this documentation tip sheet on writing porfessional interventions and actions by the person and chosen natural supporters. The final element of the person-centered plan is the interventions section which may have also heard referred to as the “methods” or “services” section. Services can include groups, interventions and individual treatment provided by clinical staff, both professional and paraprofessional. Specific examples might be: medications, psychotherapy, self-help and peer-support; exercise and nutrition guidance, Wellness Recovery Action Planning, spiritual practices and affiliations; homeopathic and naturopathic remedies, cultural healing practices/involvement of indigenous healers, and a range of rehabilitation opportunities such as supported housing, supported education, supported employment, and supported community living. One primary distinction between a more traditional treatment plan and a comprehensive person-centered plan is the inclusion of documented action steps in the person-centered plan by the person and any involved natural supporters in order to build a personal sense of self-agency as well as a robust circle of support.
