Creating SMART Objectives Tip Sheet

Review this documentation tip sheet on writing meaningful short-term SMART objectives (Specific, Measurable, Attainable, Relevant, Time-framed). When writing person-centered plans, it is also necessary to identify specific, shorter-term action steps that can help the focus person to move toward their unique life goals and dreams. These steps are usually referred to as “short-term goals” or objectives.” These are best thought of as interim goals that break down longer-term aspirations into meaningful and positive short-term changes. They should reflect a concrete change in functioning, change in behavior, or change in status, that, when achieved, are “proof” that the person is making progress. And, as with all parts of the written PCP, the short-term goal statement should be arrived after only after a thoughtful dialogue with the focus person about what they feel would be a meaningful “step in the right direction.”
