This suggested planning template is offered by NC DMHDDSUS to help providers meet PCP Guidance Document expectations that went into effect November 1, 2023. The template offers a step by step model for documenting a PCP to ensure it includes exploration of diverse life domains, person-centered interview questions, and an action plan for coordinating natural and professional supports in the service of a person’s/family’s priority goals. The document also contains a sample “mock” PCP completed in the template to further illustrate the template’s value in adhering to Guidance Document standards.
